Daily COVID-19 Screening Information for In-Person Students
Dear Parents and Guardians,
Welcome back to 2020-21 school year.
This school year, all parents are required to complete the COVID-19 Screening form daily for students receiving in-person instruction.
Each morning your child is expected to physically attend school, please sign on to the Genesis Parent Access portal. Click “Forms” then click “Daily COVID-19 Screening”. Complete the four question survey, and click Submit.
You must complete a “Daily COVID-19 Screening” form for each of your children attending in-person instruction.
The “Daily COVID-19 Screening” form will be ‘live’ in the Genesis Parent Access portal on the morning of the first day of school.
Daily COVID-19 Screening
- Are you experiencing any of the following symptoms that are not attributed to a chronic or known condition?
- Cough
- Sore throat
- Muscle aches
- Loss of smell or taste
- Shortness of breath
- Have you had close contact with or cared for someone who is currently sick?
- Have you, or someone in your household, been diagnosed with COVID-19 in the past three weeks?
- Is there any reason to believe anyone in your household may have COVID-19?
- Do you have a temperature above 100.4?
Students should stay home if they have:
- Recent close contact (being within 6 feet for at least 10 minutes) with a person with COVID-19 in the past 14 days.
- Travelled to an area with high levels of COVID-19 transmission in the past 14 days.

Please be reminded that a child exhibiting any symptoms should be kept home until they are symptom free.
