Who should complete this form?

• Incoming Kindergarten students transferring into Ewing mid-year after starting the school year in another district.
• Incoming Kindergarten students who attended a preschool program offered by a public school district.
• Incoming Kindergarten students receiving special education or related services.

**This form is not required for all incoming kindergarteners and does not need to be completed if your child does not meet at least one of the above criteria.**

Prepare the following documents to be uploaded to the Genesis Portal BEFORE starting Step 3. 

All supporting documentation must be in PDF format and uploaded at the time of registration. Photos of documents are NOT acceptable. We recommend downloading the “Microsoft Lens” app to your mobile device, which will allow you to take photos of your documents and convert them to PDF format if you do not have access to a scanner.

Required Documents for Enrollment:

Student Records:

• Birth Certificate
• Physical OR Universal Health Form (Valid for one year from date of visit) - Click here for a blank copy of our Universal Health form
• Immunization Records
• Dental Form - THIS FORM IS OPTIONAL (Print PDF and give to Dentist)
• Transfer Card/Withdraw Form, if applicable
• Individual Education Program (IEP), if applicable
• 504 Plan, if applicable
• Custody Documentation, if applicable
  • Court Custody Order or Parenting Plan
  • Notarized Letter from Second Parent on Birth Certificate (notarized letters are ONLY accepted in custody arrangements between biological parents)
  • Documentation of Guardianship (if the enrolling individual is not the parent)

Parent/Guardian Identification:

• Valid Driver's License, Passport, or State-Issued Photo ID

Proof of Residency within Ewing Township:

• One (1) of the following to show proof of Homeownership or Rental:
  • Deed / Mortgage / Property Tax / Lease / Lease Renewal / Landlord Affidavit
• Four (4) Unique Additional Proofs of Established Residency (Must show parent/guardian name and address, dated recently) such as:
  • Utility Bill (Gas, Electric, Water, Sewer)
  • Cable Bill
  • Driver's License
  • Vehicle Registration Card
  • Car Insurance Card
  • Bank Statement or Letter from Bank
  • Paystub or Letter from Employer Certifying Address
  • Homeowner's/Renter's Insurance
  • Any Recently Dated Business Mail

For Families living with a host family:

We refer to students living in a home that is not owned by their parent as “being hosted.” There is one form for the parent/guardian to complete letting the Central Registration Office know that a student is living with another family. The second form is for the homeowner/leaseholder (also called “host family”) to complete confirming that you are residing with them at their residence. Together these forms are verification that you, your student, and the homeowners/leaseholders are all living together at the same address. Both forms should be returned signed and NOTARIZED.

If you are being hosted in a rental: Click here to complete a Landlord Affidavit. The Landlord Affidavit should be completed by your landlord and/or property manager verifying your rental details. This form will confirm both the current lease term and occupants of the rental property.
