Age and Residency Requirements

Your child must be 5 on or before October 1^st to be eligible for Kindergarten and must reside with their Parent or Legal Guardian in Ewing Township.

PLEASE COMPLETE STEPS 1 & 2 TO ENROLL YOUR CHILD IN KINDERGARTEN

**Your child’s registration will not be finalized until ALL STEPS of Online Registration are complete and all documentation has been provided**

STEP 1: Required Documents

All supporting documentation must be uploaded to Genesis at the time of registering. Please prepare your documents BEFORE beginning Step 2.

• Your child’s birth certificate
• Custody Paperwork or Notarized Letter from 2^nd Parent not residing with student (if applicable)
• Physical / Universal Health Form (valid for one year from date of visit) Click here for blank Universal Health Form
• Immunization Records
• Parent / Guardian’s Driver’s License / Passport / State Issued Photo ID
• One (1) item showing Homeownership or Rental (see Acceptable Proofs of Residency below)
• Four (4) Additional Proofs of Established Residency (see Acceptable Proofs of Residency below)
• Host Affidavits: If you reside with someone who is the homeowner or tenant - both forms must be NOTARIZED: Host Affivit (Host) & Host Affidavit (Parent)
• Landlord Affidavit: This applies to renters without an updated lease, or Host Families residing with a renter Landlord Affidavit
• Dental Form - THIS FORM IS OPTIONAL (Print PDF and give to Dentist)

Acceptable Proofs of Residency:

One (1) item to prove Homeownership or Rental: Deed / Mortgage / Property Tax / Lease / Lease Renewal / Landlord Affidavit

Four (4) items establishing residency in Ewing Township that are recently dated (showing name and address) such as: Utility Bill (Gas, Electric, Water, Sewer), Cable Bill, Current Driver’s License, Car Insurance Card, Vehicle Registration Card, Bank Statement or Letter from Bank, Paystub or Letter from Employer of address change, Homeowner’s / Renter’s Insurance, Any recently dated business mail

STEP 2: Online Registration

Make sure you have all supporting documentation from Step 1 ready to upload. Genesis will NOT save your progress if you do not complete the process.
